To Become an Expert Patient

  1. The Greater Health Community Binder
    The  file is identified as 1 GHC Binder and is the master indexing file to all documents and websites.
  2. Health History Form
    This document could be your ticket to never entering all of your information multiple times to every new provider you see. This is usually more complete than any they will give you. So once you fill it out you can give it to them as an attachment into their electronic record for you.
  3. Current Medications Form
    These are the medications that your health care professional has advised you to take. Include medications, vitamins, and supplements available over-the-counter.
  4. How to Request Your Medical Records (PHI)
    Information on how to request your Personal Health Information (PHI) from your health care provider(s). The content of your PHI dictates the urgency to incorporate the new information into your GHCP.
  5. Secure File Management: All clients will choose their preferred way to store their own healthcare information:
    A.  on your computer in your “C drive” and in a folder you name,
    B. In a “Cloud” like One Drive, ICloud  or Citrix Sharefile. Most are free except the latter, Sharefile is used by TriUnity Health to send you the proprietary eBook named MyGHCP.
