The two seminal books by the Institute of Medicine (IOM), in 2000 and 2001 respectively, are “To Err is Human” & “Crossing the Quality Chasm”. They are required reading for a CCM by virtue of our certifying institution. The IOM publications informed all three branches of government of the only best estimate of deaths by medical commission or omission of 98K annually and that was based on statistics produced a decade prior to the IOM report, (i.e. 30 years ago). CCM’s more than anyone else, realize the American made for profit health care systems displaces focus on safety first. Answer: CCM’s can be strategically present at every transition of care from one entity to another going up or down vertically in acute care or horizontally from “Home”. “Independent CCM’s” affiliated with TriUnity referrals are your true patient-centered advocate and navigator for safety first as differentiated from “Hospital CCM’s” in the acute care continuum.

Congress did not respond to the reports from the IOM until 2005 in what’s known as the Patient Safety Act (PSA) of 2005.The essence of that law is first and foremost a voluntary effort for Providers to report accidental deaths caused by Providers (i.e. “iatrogenic”) or Institutions (i.e. “nosocomial”). There are no required regulatory measures to identify, quantify, report, nor interventions for uprighting the ship, thereby desecrating the original Hippocratic Oath. But:

In a report by the Journal of Patient Safety of 2013, Healthcare of America caused the deaths of no less than 250K to 400K per year, (by licensed medical providers and medical institutions). Interestingly the CDC does not include that statistic among specific diagnostic categories, they report the statistics are “classified”. Since that time, the cause of death by the American Healthcare System is not monitored because there is no mandatory law upon medical Practitioners and institutions of profit motive-based healthcare, and not even by Obamacare aka ACA.
