My Global Health Care Plan (MyGHCP)

  • All individuals may request the free Basic GHCP by calling TriUnity Health Institute 360-808-2047. You may also obtain the complete MyGHCP eBook if you start or become a member of your own Greater Health Community patient group. Please call TriUnity Health Institute and speak with Harry in assisting you and providing the latest information on diagnostic GHC self-help groups with similar diagnosis and conditions. 360-808-2047.
  • The GHCP in its many configurations to promote understanding is also know as THE LIFE AND HEALTH PYRAMID. Primary and Secondary Healthcare Providers do not require class instruction however your staff is required to obtain and pay for instruction which is reduced for ancillary staff and CCMs through 2024.

PROGRAM SERIES A

GHCP Basics of Health Self-Management (1-5); Cost $125.00. Alternatively, single class purchase fees are $35 for class one and $30 for all other classes, (2-20).

  1. Review and completion of Quick Start Up Guide composed of HA2T assessment & Patient Activation Measure ($10); GHCP scope and relationships concept with the Triunity mission; the distinction between the afflicted Greater Medical Community, The Care Model and the Greater Health Community, (pg. 3 of 1 GHC Binder).
  2. Medications management; drug–drug interactions by Uptodate.com; medication reconciliation; Health Plan and Provider resources discussion; Needymeds.org site. (Your entire program class fee could be paid by locating drug cost resources.)
  3. Diagnostics and medical documents record keeping and file management; “Nothing About You Without You”; store your PHI and Care Plan in the Cloud, your computer or on a flash drive; health institution developmental staging in relationship with PHI access.
  4. Advanced Directives review and explanation; resources; relationships to full treatment, palliative care and hospice and informed consent; state and federal laws. Three book reviews: “Families Illness and Disability”“Language and the pursuit of Happiness”“The Eight Concepts of Bowen theory”.
  5. Journal styles and worksheet review; Livescribe demonstration; how to benefit from home care agencies with the GHCP training in circumstances of physical or cognitive disabilities or end stage disease; empowering families being informed and leading and engaging in your loved ones C3 from afar.

PROGRAM SERIES B

GHCP Expert Patient & Informal Caregiver (6-10); Cost $125 ($250 for total of A, B). Alternatively, single class purchase fees are $30 each class.

6. My health clinic and hospital characteristics and the Global Health Care Plan. Institutional and provider report cards; The Chronic Care Model and newly revised Care Model; Patient Safety.

7. My Primary Caregiver characteristics and caregiver burden; MYDECAF; Genogram.

8. Community Care Matrix Pathway; The Community Care Setting Matrix; Care Transitions.

9. Planned Care; Deep Dive and the CGBAO’s; Shared Community CGBAO’s.

10. Global Skill Development: learning triangles, interlocking triangles, communication distancing and cutoffs; dyad instability as gradations of: “Conflicting, Defensive, Stonewalling, or Contemptible” interactions and behaviors.

PROGRAM SERIES C

GHCP Master Learning Coach Program (11-20); Cost $250 ($500 for total of A, B, C). Alternatively, single class purchase fees are $30 each class.

11. Integrating multiple conditions management with the Psycho-Social Typology of illnesses. (See Care Transitions Excel spreadsheet).

12. Coordinating your care with input from the Shared Community CGBAO’s and specific disease community group forums.

13. Maslow, Branden, and Royce and Life Plan Self Efficacy, (the left side of the Health Pyramid).

14. It’s time for the Dreaded Drama Triangle- understanding how things don’t get fixed or gets worse; comparing and contrasting the TED model to the Life Pyramid Paradigm.

15. Basic Self and Pseudo Self and establishment of relationship boundaries within the Life Force and Togetherness Force, a part of Bowen Family Theory; change requires stepping outside of ourselves and seeing where we are in our relationships what we are doing and how we can establish a new action or movement (i.e. non-reactionary) towards lasting but stable and healthier change; a new movement requires strategizing or simply letting go or neutralizing emotions, (et.al.), to leap frog from Triangles to Triads and a new stability that lasts.

16. Life Style Change; Diet & Exercise creating new habits using the GHC and the 5F’s.

17. Using the GHCP during acute illness in transitions of care and in health maintenance; monitoring and evaluating your C3.

18. Cognitive dysfunctions a global deactivation and boundary issue; Global Deterioration Scale & MMSE; Consequences to caregiving and family relations and responsibilities; Caregiver preparedness and strain; MYDECAF; community care setting re-emphasized.

19. Reviewing your GHCP, group learning and community participation for the greater health of our community.

20. Integration: individual, family and community outcomes and projects. Re-evaluation what are your PAM and HA2T now?

All class fees apply to online using Adobe Connect. Regional and neighborhood classroom sessions by arrangement are encouraged in the Counties of  Kitsap, Mason, Jefferson & Clallam. Please refer to Events tab for scheduling of online and office classes. Please also refer to the Home tab and the “Introduction” document in the dropdown and go to “Requirements” sub heading to see what hardware and software you need to use the GHCP.

Classes are by demand—see descriptions below. Class minimum attendance offerings are: online, and office classes of = to or > 8 persons; neighborhood or regional visits are determined by travel distance (each student may bring one GHCP manager or caregiver); 1:1 online assistance is offered telephonically at a pre-paid rate of $80 per hour counted in minutes of each call.
