- Professional 1:1 Health Coach Consult with Patient — using MyGHCP guidelines — $80/hour Pre-paid one hour counted in five minute increments, (offered M–F 8am–5pm PST), this consulting is beyond or instead of classroom content and patient specific with directions or guidelines within the GHCP resources .Assume by phone calls or texting or specify preference.
- Acute Episode –Deliberate-Transitions “Hospital to Home” Interventions and education GHCP Programs A & B with one hour Face to Face (Home or Facility) and two hours telephonic & online (patient and caregiver will complete HA2T & PAM self-assessments) — $250.00 — Approximately two week interventions
- Add Telephonic/online 24/7 consults — outside of office hours — $120/hour Prorated in 15 minutes, call 360-808-2047 (non-office hours) Additional Home or facility appointments — $120/hour with one hour minimum then prorated in Kitsap/Clallam/Jefferson/ or Mason counties plus $40/hr commute time Reference: see 4 a) and b) pg. 4-5 in GHCP Introduction document for descriptions
- Persons calling for CCM guidance in your community is always preferred, rural areas will always be in most cases a problem to where you may have to seek CCM’s from out of your county in rural areas.Those CCMs will be aware of this gap and they will know all medical services and seek the nearest providers for your specific area. It may necessitate travel to certain specialists for that care, all providers will provide zoom calls when direct examination is not warranted. Triunity will provide remote zoom offering at your preference. Healthcare Provider - Certified Triunity Health Coach Professional Training — MyGHCP GHCP Programs A, B & C — $500/person — related to ii. Above usually when alternate transfer to SNF a AL or AFH temporary placement needing Nurse case management to prevent delays and lack of appropriate provider follow ups. Group discounts with 5 or more licensed professionals — $450/person
